TheDebt Referral Questionnaireis a form a referring agency completes for each debt type before referring debt to the Minnesota Department of Revenue. The form requests information about the agency, debt type, payment application, and method of referral.
All referring agencies, except for the Minnesota Judicial Branch, must send anotice to debtorby U.S. mail or personal delivery at the debtor’s last known address at least 20 days before referring a debt to the Minnesota Department of Revenue for collection. The notice must include:
The nature and amount of the debt
To whom the debt is owed
Actions we may take to collect the debt
Notification of our collection costs
Their rights to ask for a cancellation of collection costs
Notification that interest-bearing debts continue to accrue interest
We want to ensure referring agencies successfully administer debts referred to the Minnesota Department of Revenue for collection. All administrators and authorized users who manage referred debt must complete mandatory training before we grant access to e-Services.
