Submit Contractor Affidavit Electronically

• A supported web browser ( See Minnesota e-Services System Requirements)
• The beginning and ending dates you worked on the project
• Project number
• Total contract amount
• Project location
• Project owner
• Project owner's complete address
• The name and address of the company hiring you (if you were not hired by the project owner)
• Approved electronic Contractor Affidavits from your subcontractors

1. Log in to your e-Services account
2. In the “I Want To” tab in the Contractor Affidavit (IC-134) panel select, Submit contractor affidavits

If we approve your Contractor Affidavit, you will see a confirmation page. Send this page to the person who hired you.

Note: If you are a single-member corporation, you are considered an employee of your own business. For details, see Withholding Fact Sheet 6 – Corporate Officers.

To correct errors on a Contractor Affidavit you submitted, complete a new Contractor Affidavit with the correct information. We will compare your original Contractor Affidavit with the new one and note any changes.

Once approved, we will provide you a new affidavit number (or approval letter, if you filed by paper). Provide the new affidavit number or letter to the person who hired you.
