Form SBR Instructions


Each qualifying volunteer firefighter relief association that paid supplemental benefits to qualifying firefighters in calendar year 2023 must complete Form SBR no later than February 15, 2024, to receive reimbursement in March 2024.

Important Notes

  • You may also submit previously unclaimed supplemental benefits paid in calendar years 2019 to 2022.
  • Each relief association can submit only one form per filing period (mid-November to February 15 each year). Be sure to include all firefighters on your form.
  • Verify your form has been received. You can find more information about this under the Certification section.
