All courses must be board approved. If a course is not listed on the board approved lists, the Request Form for Course Approval will need to be completed and submitted for board approval.
All licensed assessors must submit their request for course approval on the Assessor Licensing System. Note: The course outline and description of the course along with a timeline anticipated for each topic must be included in the request
Exam Proctor Affidavit - If the online sponsoring organization does not provide a proctored exam the Exam Proctor Affidavit must be completed and sent in with the completion certificate. Under board policy, the person proctoring an online exam for licensure must be a disinterested third party with no conflict of interest who verifies a student's identity and processes a board approved affidavit that the student received no outside assistance.
