Proctor Affidavit for Exams

If the online sponsoring organization does not provide a proctored exam, a proctor affidavit is required with the course completion certificate. View or download the Exam Proctor Affidavit form.

The student must:

• Arrange for a proctor to monitor their exam.

• Submit the affidavit — signed by both student and proctor — with the course completion certificate.

The proctor must:

• Be a disinterested third party with no conflict of interest.
• Verify the student's identity.
• Monitor the student during the exam.

• Certify the student received no outside assistance.
