About This Page

Fire State Aid and Peace Officer State Aid help subsidize pension costs for local governments and fire departments. You must submit Form PA-1, Certification of Peace Officers or Form FA-1, Fire Equipment Certification, to the Department of Revenue by March 15 to avoid losing part or all of your aid for this year.

Note: The online submission form we expected to be available for the PA-1 in January 2013 will not be available in time for use this year. Please submit the paper form you received or by using the fillable PDF below.

Instructions and Forms

Penalties

If we do not receive your forms by March 15 we will send a final reminder stating that your fire or police department could forfeit part or all of its aid for this year if we do not receive your forms within 10 days. The amount of aid forfeited is equal to the amount of this year’s aid multiplied by 5 percent for each week or fraction of a week that the form is late.

Changes in service areas and apportionment agreements

If your fire department has had any changes in service areas, you must complete an apportionment agreement and file it with the department. Please use the examples and forms below to change the percentages used in determining your state fire aid.
