Referring Agency Resources

The Debt Referral Questionnaire is a form a referring agency completes for each debt type before referring debt to the Minnesota Department of Revenue. The form requests information about the agency, debt type, payment application, and method of referral.
All referring agencies, except for the Minnesota Judicial Branch, must send a notice to debtor by U.S. mail or personal delivery at the debtor’s last known address at least 20 days before referring a debt to the Minnesota Department of Revenue for collection. The notice must include:
- The nature and amount of the debt
- To whom the debt is owed
- Actions we may take to collect the debt
- Notification of our collection costs
- Their rights to ask for a cancellation of collection costs
- Notification that interest-bearing debts continue to accrue interest
- Revoking professional licenses

The following reports are available in your e-Services account:
- Collection Cost Removals and Other Adjustments
- Debts impacted by an adjustment, such as cancelation of collection costs.
- Inventory Report (monthly) by Referred Date or Debtor Name
- Debts we are currently collecting for your agency.
- Payment Detail Report (PDR)
- Summary of events on debtor accounts and payment details for transactions affecting debts during the report cycle.
- Rejected Transaction Report
- A list of requests, such as referrals, payments, and recalls an agency submits and we reject.
- Returned Debt Report
- Debts you recalled or we returned to you.

The Referring Agency Service Level Agreement outlines the use and participation in the Client Relations program. It is an agreement between the agency and the Minnesota Department of Revenue.
Your agency must complete a new Service Level Agreement to be able to refer debts for collection.
This agreement replaces any agreement, either formal or informal, that you currently have on file with us.

Who should sign the agreement?
The authorized agent should sign the agreement.
Who is my authorized agent?
The authorized agent is someone within your agency who may:

Have authority to make decisions or sign contracts on the agency's behalf

Supervisor employees who refers debts

Be responsible for resolving issues related to the agency's administration or participation in the Client Relations program.
The authorized agent who signed our agreement is no longer with our agency.
Is our agreement still in place?
Yes, your agreement is still in place. You should contact the Client Relations program with the new administrator's information so we can update our records.
We want to ensure referring agencies successfully administer debts referred to the Minnesota Department of Revenue for collection. All administrators and authorized users who manage referred debt must complete mandatory training before we grant access to e-Services.
This is a self-paced, computer based training (CBT). It contains audio with an option for closed captioning. We recommend viewing the CBT in one of the following browsers for keyboard tabbing and for the best training experience:

Before you begin
Make sure you have your agency name and ID number. You can:
- Find this information in e-Services
- Ask your agency administrator
- Use New Agency – 0000000 if you are a new agency
Allow 40 minutes to complete the training.
- Use your computer audio to control the volume
- Pause the training, if needed
- Start the training over if your browser closes or you leave the training
Note: You cannot return to a lesson at a later date.
Take the training
- Go to Debt Collection Act Training.
- Watch and complete the video training.
- Fill out and submit the form at the end of the training.
