You should notify the Minnesota Department of Revenue if your address or name changes after filing a tax return.
Address Change
To change your address, you may either:
- Call 651-296-3781 or 800-652-9094
- Email us at individual.incometax@state.mn.us
If you email us to change your address, include:
- The last four digits of your Social Security Number
- Your full name
- Your date of birth
- Your previous mailing address
- Your current mailing address
- Your phone number
Name Change
Notify us in writing if your name changes or does not match the name we have in our system. Include a copy of any of these examples of legal documentation:
- Valid current driver's license and previous driver's license that show the change
- Social Security card
- Divorce decree
- Marriage license
- Form N-400, Application for Naturalization
- Any court document identifying a name change
