Submit Levy Disclosures and Payments

If you receive a levy disclosure request from the Minnesota Department of Revenue, use the following table to determine if you must send the required information electronically.

​Bank ​You can send us the required information electronically in e-Services.
​Wage ​You must send us the required information electronically in e-Services.

You have two options when submitting your levy disclosures electronically. They are:

  • Manually enter levy disclosures
  • Import an XML file of levy disclosures

To determine the best method, please review the different options below.

Manually Enter Levy Disclosures 

You can manually enter in your levy disclosure information. To do so:

  1. Log in to your e-Services home page.
  2. Select Submit a levy disclosure in the I Want To section.
  3. Select Next under the Manually Enter your Levy Disclosure Data option.
  4. Complete the remaining steps as directed.
Note: You will have the option to make a payment while submitting levy disclosure information.

Import an XML File 

You can import an XML file containing the levy disclosure data. The XML file has a specific schema that must be used so the file is processed correctly.

For additional information on the file format, please see the Levy File Format page.

  1. Log in to your e-Services home page.
  2. Select Submit a levy disclosure in the I Want To section.
  3. Select Import Your Data under the Import Your Data option.
  4. Select the file you are importing.
  5. Click Next.
  6. Complete the remaining steps as directed.
Note: You will have the option to make a payment while submitting the levy disclosures.

Payment Options

You have the option to make a payment with your levy disclosures. If you want to make a payment at a later date, you have several methods you can use to submit the payment. Use the links below to determine which method is best for you.
