  • Can I receive certification of my contractor affidavit today?Back to top

    Yes. If you complete your contractor affidavit electronically (online), you will receive immediate notification of the status. If your contractor affidavit is approved, you will receive a confirmation.​

  • When filing my contractor affidavit electronically, why do I get an error on Step 3?Back to top

    The most common errors are:

    • You entered the subcontractor's federal ID number when you should be entering their account number. The account number is listed on the confirmation page below the affidavit number.
    • You did not complete all fields marked with an asterisk (*). These fields are required.
    • You did not include all nine digits of a Social Security number. To get the subcontractor's Social Security number, see their Form W-9, Form 1099, or contact them directly.​

  • Why can't I find a contractor affidavit number on my subcontractor's paper Form IC134?Back to top

    All paper IC134 forms are certified by a signature and date stamp. Paper IC134 forms do not receive a contractor affidavit number.​

  • Can I change/edit my electronic contractor affidavit?Back to top

    No. You must complete a new electronic contractor affidavit with the correct information. You will also get a new affidavit number. Be sure to give the new contractor affidavit to the person who hired you.​

  • Where is my paper Form IC134?Back to top

    If you submitted your contractor affidavit using Form IC134, approval can take 4 to 6 weeks. You cannot receive final payment until your Form IC134 is certified and returned to you. For faster service, you can complete an electronic contractor affidavit, even if you already sent in a paper Form IC134.​

  • How will I know when my contractor affidavit has been certified?Back to top

    If you submit your contractor affidavit electronically, you will receive immediate notification. If it is approved, you will receive a confirmation.  Print and keep the confirmation page for your records.  To receive your final payment, you must give a copy of the confirmation page to the governmental unit or the prime contractor that hired you. 

    If you complete your contractor affidavit using Form IC134, approval can take 4 to 6 weeks. Once approved, the form will be signed and returned to you.  Keep a copy for your records. To receive final payment, you must give a copy of the certified IC134 form to the governmental unit or the prime contractor that hired you.  

  • I didn't print (or lost) my contractor affidavit, how do I get another one?Back to top

    If you submitted your contractor affidavit electronically using e-Services, log into your withholding tax account and choose the History tab to view previous contractor affidavits.

    If you submitted your contractor affidavit electronically in e-Services using your Social Security number, check your email box for instructions on how to view your contractor affidavit. Instructions were emailed to you when you applied for your contractor affidavit.

    If you no longer have the email, go to our website and click on Contractor Affidavit information for Government Projects. Then click "Submit contractor affidavit electronically using e-Services."  Select "No" to both questions on the e-Services page that displays.  Then select "View Approved Affidavit".  You will need your email address and the confirmation number. If you do not have your confirmation number, you must submit another contactor affidavit.​
